The therapy dog stopped just inside the hospital entrance and lifted his nose.
His handler smelled the familiar things: disinfectant, coffee drifting from somewhere beyond the lobby, perhaps the faint medicinal odor that seems to cling to every hospital corridor. The dog was receiving an entirely different version of the building. He sniffed the air once, then again.
A nurse hurried past carrying a paper cup. A family stepped from the elevator. Someone had recently eaten lunch on a bench near the window. Another dog had crossed the lobby hours earlier. Shoes carried traces of rainwater, pavement, grass, homes, other animals, and places the handler would never know existed.
To the handler, they were walking through a hallway. To the dog, they may have been walking through information.That difference between the canine and human experience is becoming increasingly fascinating as researchers explore olfaction, the sense around which much of a dog’s world is organized. Scientists have already demonstrated that dogs can distinguish odor samples collected from people before and after experimentally induced psychological stress. That does not mean a therapy dog can diagnose anxiety, read someone’s thoughts, or automatically know which patient needs comfort. It means human physiological changes can alter odor in ways dogs are capable of detecting. (PubMed)
Research published in Scientific Reports in February 2026 pushed the question further. In a study of 43 dogs, researchers presented odor samples collected from unfamiliar dogs following joyful, stressful, and relaxing experiences. The dogs distinguished certain emotional odor categories and changed aspects of their behavior depending on what they smelled. Exposure to stress related odor was associated with greater proximity to their owners and less proximity to strangers. (Nature)
The study involved dogs smelling other dogs, not therapy dogs identifying emotions in hospital patients. That distinction matters. Still, it offers a remarkable glimpse into how much communication may be occurring in a sensory world largely unavailable to us.
Later that afternoon, the therapy dog entered a patient’s room. The woman in the bed smiled immediately. “ Oh, look at you.”
Instead of walking directly to her, the dog lowered his nose and investigated the floor near the doorway. He sniffed the leg of a chair, moved toward the bed, paused beside a visitor’s coat, then finally approached the woman’s outstretched hand.
Years ago, a handler might have been tempted to hurry him. “ Go say hello.”But perhaps sniffing is not wasted time. It may be how a dog enters a room.
We humans look around. We notice faces, furniture, expressions, lighting, and movement. Dogs gather much of their information through scent. Allowing a therapy dog a few moments to investigate an unfamiliar environment may therefore be less like permitting a distraction and more like allowing someone to look around before beginning a conversation.
There is another reason for humility. Humans are not nearly as capable of reading this hidden chemical language. A 2024 study asked dog owners and nonowners to distinguish odor samples collected from relaxed and stressed dogs. Neither group could reliably do it. Perhaps that should change the way we think about therapy dog visits. We enter the same hospital together, but we do not enter the same world.The handler sees the trembling hand waiting beside the bed.
The dog may notice things carried on breath, skin, clothing, and air that the handler will never perceive. Neither possesses the whole picture. And somewhere between the world we can see and the world they can smell, two species have learned to walk beside one another, trusting each other enough to share what neither could experience alone.
